Dementia and Aged Care Services (DACS) is a trusted community aged care provider supporting elderly Australians to live independently, safely, and with dignity in their own homes. We support clients aged 65 years and over, with needs ranging from low-level assistance to more complex care requirements.
We are seeking an experienced and client-focused Client Services Coordinator to lead the coordination of Commonwealth Home Support Programme (CHSP) services and Support at Home (SAH) team across the Tamworth- New England region. This is a permanent full-time position based at Tamworth NSW office.

You will work closely with clients, families, community partners, and support staff to ensure services are responsive, well-coordinated, and aligned with individual goals and funding requirements.
This role requires strong organisation, empathy, accountability, and the ability to balance operational coordination with genuine client care and key responsibilities as below
- Coordinate and manage CHSP services across the Tamworth- New England region
- Build relationship with clients and develop individualised support plans
- Monitor service delivery to ensure quality, compliance, and positive client outcomes
- Maintain accurate and timely documentation in line with requirements
- Support the recruitment and coordination of community support staff
- Build strong, respectful relationships with clients, carers, and local service providers
- Contribute to community engagement and service development initiatives.
Successful candidate for this role will demonstrate as below:
- Qualifications in Aged Care, Community Services, Social Work, or a related field
- Demonstrated knowledge of CHSP and SAH funding models and compliance requirements
- Experience coordinating community-based aged care services
- Strong organisational, communication, and problem-solving skills
- Ability to work independently while contributing to a collaborative team environment
- Current Driver’s Licence and access to a reliable vehicle
- Valid Police Check
