Position Overview
The Care Coordinator plays a crucial role in delivering person-centered case coordination for our Consumer Directed Care Package consumers. You will conduct initial intake assessments, develop and maintain comprehensive care plans, and provide holistic case management while closely collaborating with the Case Manager, consumers, and their families.
Your responsibilities will also include applying the principles of wellness and reablement, ensuring that care coordination aligns with best practice standards, legislation, and DACS's philosophy, policies, and procedures.
Ideal Candidate Profile
We are seeking dedicated candidates who possess a genuine passion for aged care and a commitment to enhancing consumer outcomes.
- Demonstrated experience in Aged Care, particularly in a Care Coordinator or similar role.
- Ability to work autonomously while effectively prioritising and managing tasks.
- Strong communication skills to engage with older individuals, families, and team members regarding consumer care.
- Relevant qualifications in the field of aged care or a related area.
- Extensive knowledge of Home Care Packages.
- National Police Clearance Certificate.
- Current First Aid & CPR Certificate.
- Yellow Card (or willingness to obtain one).
- Current influenza vaccination and a willingness to receive annual vaccinations.
- A valid Open Drivers License and a registered motor vehicle in Queensland with comprehensive insurance.
As a Care Coordinator, you will use your expertise and evidence-based approaches to deliver quality, coordinated interventions tailored to the specific needs of Home Care Package consumers.
